Leukaemia

A group of cancers which originate in the bone marrow with high number abnormal white blood cells are known as Leukaemia or blood cancer. Generally bone marrow contains of few healthy and many immature blood cells which are known as blast cells. Under certain conditions, the genes that carry information for the multiplication of cells in the bone marrow undergo mutations and/or epigenetic changes and they start growing and multiplying very fast in an uncontrollable manner. This abnormal production of cancerous cells in bone marrow interferes in the normal blood cell production in the bone marrow and is released to the blood circulatory system with a mixture of mature and immature cells.
There are mainly 4 types of blood cancers. They are Acute Myeloid Leukemia (AML), Chronic Myeloid Leukemia (CML), Acute Lymphoblastic Leukemia (ALL) and Chronic Lymphocytic Leukemia (CLL).
The speed at which leukemia develops and progresses are referred by the terms acute and chronic. Acute Leukemia’s develop and progress fast, whereas chronic leukemia’s develop and progress slowly. Based on the basic stem cells from where the leukemia develops is the basis for classifying as myeloid and lymphoid leukemias. There are different types of white blood cells in human beings, in which the B – Lymphocytes and T – Lymphocytes originate from lymphoid stem cells. When leukemia originates from lymphoid stem cells, ALL and CLL are developed. All other types of white blood cells like neutrophils, basophils, eosinophils, monocytes, etc. are originating from myeloid stem cells and when leukemia starts from myeloid stem cells, then AML and CML develops. Myeloid forms of leukemia are further classified into different subgroups based on which individual type of white blood cells of myeloid stem cell is affected.
Among different types of leukemia, chronic leukemias are generally common among adults and acute leukemias develop both in children and adults. But ALL are more common among the children.

Generally Known Causes of Leukaemia

Material science based medical science has no clear information about the causes for the development of leukaemia. But from experience it has been found that certain factors have serious effects on the formation of Leukaemia or blood cancer. They are known as risk factors and are detailed below:

  • Exposure to chemotherapy and radiation therapy
  • Exposure to chemicals like benzene
  • Intake immunosuppressant medications such in the case organ transplant
  • Intake of corticosteroids
  • Hereditary tendency for blood cancer
  • Genetic disorders like Down syndrome, Fanconi anemia, etc.
  • Presence abnormal chromosome no.: 22, also called Philadelphia chromosome
  • Presence of HIV / AIDS

Signs and Symptoms of Leukaemia

The common signs and symptoms of Leukaemia include:

  • Paleness
  • Tiredness
  • Shortness of breath
  • Excessive bleeding
  • Increased susceptibility to infections due to decreased immunity, etc.

There can be enlargement of lymph nodes, spleen, and liver. Most of the patients do not experience any symptoms during early stages of the disease.

Diagnosis of Leukemia

In general SGHHCC and SGCCRC consider all available options for the diagnosis of the cancer. We always prefer non-invasive methods than invasive methods. If invasive methods are highly necessary, then next priority will be to prevent the spread of the cancer as a by-product of the diagnostic procedure. As a general rule, we appreciate the biopsy to be done after total surgical removal of the entire tumour or affected organ with all nearby enlarged and affected lymph nodes so that the cut ends of the malignant tissues will not become instrument for the spread of the cancer inside the body.

Preliminary diagnosis of leukaemia usually takes place during routine blood test. Later one or more of the following tests will be conducted to confirm the diagnosis.

  • Physical test: Physical examination is conducted to find out swollen lymph nodes, spleen and liver.
  • Blood test: Blood tests are conducted to count the number of red blood cells, platelets, and white blood cells.
  • Biopsy: In this test, samples of tissues of bone marrow are examined under a microscope to ascertain whether leukemia cells exist.
  • Cytogenetic testing: This test is conducted on the samples blood, bone marrow and lymph nodes. This test finds out the existence of abnormal chromosomes in the blood and it also identifies the type of leukaemia.
  • Spinal tap: This test is conducted by taking spinal fluid from the lower spine area and is to test the presence of leukaemia and signs of other problems
  • Chest X-ray: Chest X-rays are sometimes taken to identify lymph nodes and diseases in the chest
